 A seafarer had been instructed to help with the final cleaning of a hold that had been loaded with sand. The cleaning was carried out with a broom and a shovel and the operation was watched from above by the Third Officer. Because of the poor light in the hold, the seafarer decided to use a light cluster placed close to the top of the hold ladder to illuminate the whole space. When the cleaning was completed, the seafarer started to climb the long, mostly unprotected, ladder. When he was close to the top, the seafarer fell to the ground. The Third Officer heard a cry and a thud on the floor of the hold, looking down, he saw the seafarer lying at the bottom of the hold. He immediately went down into the hold to ascertain the injuries to the seafarer and then, because of the victim's serious condition, called the health emergency service.
